Wound care that shows up.
Highly experienced nurse practitioners come to the bedside, wherever the patient lives: skilled nursing, assisted living and memory care, board and care, congregate living and private homes, across ten Southern California counties.

How can we help?
Choose the path that fits you. Each one opens what matters for your role, or for someone you care for.
Wherever the patient lives.
The same experienced clinicians and the same written note in every setting. Choose one to see who we work with there, and who carries out the care between our visits.
Patients in these settings can be referred too. Call our office, and we confirm availability for each one.
Wounds, explained.
Plain-language guides for families and care teams: what each wound is, who is at risk, which changes to report, who is part of the care, and where to read more from official sources.
General information, not medical advice. The patient's physician directs the plan of care, and whether a visit is right for a particular patient is a clinical decision made by clinicians. For an emergency, call 911.
Wound care, A to Z.
Plain-language reads from MedlinePlus, the CDC and HHS, at an easy pace with soft music. Sources are listed under each read, and our own tips are marked.
How wounds heal
Soft music, and the words move at an easy reading pace. Pause any time.Read it as text
General information, not medical advice. The music is original, written for this page and played by your browser; nothing is downloaded. It starts when a read plays and fades when you leave; turn it off with the note button.
What a visit is like.
Calm, unhurried and clearly explained, from the first call to the note that follows every visit.
The note you receive.
There is no patient here and no clinical content. Each field names what the clinician records there; select one to see where it goes. Before the first visit, we walk your care lead through this layout and adapt it to your setting.
Sample layout, not a patient recordIn a facility, the note goes to the facility record and the physician. At home, it goes to the physician and the family contact the patient has named. Any home health or hospice agency involved receives it too, and every note names the channel it was sent through.
What you can count on.
Agreed in writing before the first visit, in every care setting and with every family.
The referral desk.
Every referral follows the same four steps, whoever sends it and wherever the patient lives. Call first, and we confirm a secure channel with your team.
Alongside everyone who cares for the patient.
Facilities, physicians, agencies and discharge teams each work with us a little differently. Choose yours to see how we work alongside you, what we ask, and what we never do.
Southern California, from San Diego up.
Ten counties, from San Diego and Imperial up to San Luis Obispo and Kern. Select a county to see its cities and major routes, or check a city or ZIP code. Our office confirms every facility and home address before the first visit.
Runs in your browser. Nothing you type is stored or sent. Please do not enter a patient's name or any health information.
County lines and major routes from public map data (Natural Earth), simplified; towns and place names are shown for orientation only. ZIP codes can cross county lines, so our office confirms each address.
Coverage, one patient at a time.
Whether care is covered, and under which benefit, depends on the patient, the setting and the plan. So our office answers it for each patient, in writing, before anything is scheduled.
Four measures, defined before we count.
A measurement framework is a commitment to a method, not to a number. Turn a card to read exactly what it counts, who it includes and where the figure comes from.
Each measure is counted by calendar month and reported quarterly, and shared with the care setting or family it concerns. When a definition changes, we record the date the change took effect.
Experienced wound care, wherever the patient lives.
Elevate Wound Care was founded on a simple conviction: a patient with a wound deserves the same experienced, consistent care whether they live in a skilled nursing facility, a residential care home or their own home. The setting changes. The standard does not.
The same standard everywhere
The same experienced clinicians and the same careful notes, whether in a skilled nursing facility, a six-bed board and care home or a patient's own living room.
Agreed in writing
A named contact, a written note after every visit, and word of any change as soon as we know it. We agree on all of it in writing before care begins.
Alongside everyone
We work with the patient's own physician, the facility or home staff, the family, and any home health or hospice agency already involved.
Questions people ask.
Plain answers about how we work. For clinical questions, please ask the clinician or physician who follows the patient.
Start with a short call.
We'll listen first, then walk you through how visits, scheduling and communication would work for your facility, your residents or your family member at home. When you call, you reach a person at our office.
Monday through Friday, 10 a.m. to 4 p.m. Pacific. Closed on New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day and the day after, and Christmas Day. After hours, on weekends and on holidays, call the same number. In an emergency, call 911. Get ready for the call
For business questions only. Please never send patient names, records or photographs by email.
Administrative fax only. Please do not send patient information or referrals by fax.
Our office is a coordination line, not an emergency line.
